Terms and Conditions:
As the parent/legal guardian of the minor named herein above, I hereby agree to release and on behalf of myself and the minor named above, my heirs, representatives, executors, administrators, and assigns, HEREBY DO RELEASE The LAB Sports Academy, Inc., its owners, officers, directors, employees, volunteers, agents, representatives, insurers and affiliate entities (“Releasees”) from any causes of action, claims, or demands of any nature whatsoever including, but not limited to, claims of negligence, which I, together with the named minor, my heirs, representatives, executors, administrators and assigns may have, now or in the future, against the Releasees on account of personal injury, property damage, illness, death or accident of any kind, arising out of or in any way related to the use of the The LAB Sports Academy, Inc. facilities/equipment, participation in The LAB Sports Academy, Inc.-sponsored activities or other activities on the premises or under the direction of The LAB Sports Academy, Inc.’s personnel, whether that participation is supervised or unsupervised, however the injury, illness, or damage may occur. In consideration of the named minor’s participation in The LAB Sports Academy, Inc. activities, I, the undersigned parent/guardian of the named minor, agree to INDEMNIFY AND HOLDHARMLESS Releasees from any and all causes of action, claims, demands, losses, or costs of any nature whatsoever arising out of or in any way related to the named minor’s participation. Moreover, I expressly agree that this Agreement is intended to be as broad and inclusive as permitted by the laws of the State of Georgia.
I further understand that The LAB Sports Academy, Inc. does NOT provide medical insurance for my child and that I should consult my child’s physician before allowing my child to participate in this program/activity. In the case of accident or illness, I hereby authorize the program/activity staff to administer or seek medical treatment for my child, as they see fit, including routine first aid care or emergency medical treatment. I hold harmless and agree to indemnify the Releasees from any claims, causes of action, damages, and/or liabilities arising out of or resulting from said medical treatment. I acknowledge that I am solely responsible for any urgent care, hospital or other costs arising out of any bodily injury or property damage sustained through the minor child’s participation in The LAB Sports Academy, Inc.’s activities and programs.
I also give my permission for the use of the name, image and/or likeness of the minor child named above by The LAB Sports Academy, Inc. I understand that the images may be used in print publications, online publications, game film, video, presentations, websites, and social media and that I have no legal right to review any such material/content in advance of publication. I also understand that no royalty, fee or other compensation shall be owed to me or the minor child named above by reason of such use.
